  - उपचार से संबंधित सभी सेवाएँ जैसे-दवाई, डॉक्टर का परामर्श, बेड, भोजन, सर्जन, ओटी और आईसीयू आदि निःशुल्क है।

योजना का लाभ कहाँ लें?

- - देशभर में लगभग 28000 सरकारी और निजी अस्पताल योजना के अंतर्गत सूचीबद्ध हैं।
  - बिहार राज्य में स्थित सभी सरकारी अस्पताल तथा चिकित्सा महाविद्यालय एवं अस्पताल योजना के अंतर्गत सूचीबद्ध हैं।

- - आप अपनी पात्रता की जानकारी के लिए वेबसाइट www.mera.pmjay.gov.in देखें या टोल फ्री नंबर 14555 या 104 पर कॉल करके पता कर सकते हैं।

- - पारिवारिक पहचान के लिए राशन कार्ड या प्रधानमंत्री जी का लाभार्थी परिवार के नाम पत्र।
  - व्यक्तिगत पहचान के लिए आधार कार्ड या सरकार द्वारा मान्यता प्राप्त अन्य फोटो पहचान पत्र।

- - प्रधानमंत्री जन आरोग्य योजना के अंतर्गत 10 करोड़ से अधिक परिवारों को लाभ मिलेगा|
  - अपने मोबाइल नम्बर से लॉगिन कर पता करें आपका परिवार प्रधानमंत्री जन आरोग्य योजना में सम्मिलित है या नहीं|
  - प्रधानमंत्री जन आरोग्य योजना का लाभ लेने के लिए आपको कोई आवेदन करने की ज़रूरत नहीं है|
  - अगर आपका परिवार प्रधानमंत्री जन आरोग्य योजना लिस्ट में सम्मिलित है तो आप चिकित्सा उपचार के लिए किसी भी सूचिबद्ध अस्पताल में प्रति वर्ष 5 लाख रुपये तक का लाभ उठा सकते हैं|
